Designer or design name of this chair?

I sat on this chair in a museum cafe and I fell in love.  I must absolutely have one for our modern home.

Does anyone know the original designer or design name?
Where I can buy one now?

Still desperately seeking identification. 

That is indeed lovely....and looks really comfortable as well. Hope someone knows who designed it and where you can get it from cause I'd sure love to have one at home!
